First, make a new document that is 155x34 pixels. Grab the Rectangle Tool and make a rectangle the size of your canvas. Rasterize the layer and right click it. Choose the "Blending Options" option and enter these settings:


To edit the gradient, double click on the color. A new window should open. Enter in these colors. (The color on the ends are black, #000000.)


Now go to the "Stroke" tab.


Again, edit the gradient.


You should now have something that looks like this:


Now, make a new layer and select an airbrush. With your brush, select a blue color and make somewhat of a circle. Next, grab the Dodge Tool and set the range to "Highlights". Draw over what you brushed before. After all that, you should have something like this:


Add a white rectangle to the top of your button like this:


Rasterize the rectangle and then go to Filter> Blur> Gaussian Blur and set the radius at 8 pixels.

Finally, add your text. Once you are done with that, put the text layer under the white rectangle layer. The finished product should look something like this:
