To create forms go to your toolbar > Insert > Form.

Dreamweaver Forms, easy and quick to create with the short cut panel:

In Order:

Text Field
Hidden Field
Text area
Checkbox
Radio Button
Radio Group
List/Menu
Jump Menu
Image Field
File Field
Button
Label
Fieldset
